Health or Medical Problems Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the pet's owner upon signing this contract/agreement. We DO NOT take females in heat. We also DO NOT express anal glands, as we feel that is a medical procedure to be done by your vet. We also do not pluck hair out of the ears (we trim it short). If you would like them plucked, this form releases us from any infections or irritations that may occur from this procedure.
Accidents Although accidents are very rare, there is a risk when dealing with animals. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when an animal is wiggling or moving around. Your pet’s safety and comfort is our number one priority. In the event an accident does occur, you will be notified of the accident. If Pooches N’ Purrs feels it is serious, and the owner is not on-site, Pooches N’ Purrs will seek immediate veterinary care for your pet with our neighbor, Family Pet Clinic.
Veterinarian Authorization-Medical Emergencies This release gives Pooches N’ Purrs full authorization to seek medical treatment from our neighbor, Family Pet Clinic, in the case of any medical emergencies while in the care of Pooches N’ Purrs.
Vaccinations All pets are strongly recommended to be current on all vaccinations. They are REQUIRED to be up to date on their Rabies vaccination as it is the Illinois State Law (we call your vet to check) in order to receive grooming services. Thank you for your understanding.
Bordetella Vaccination Kennel cough (similar to a human cold) is a respiratory condition caused by a variety of viruses and bacteria often spread by direct contact by dogs. At Pooches N’ Purrs we do disinfect between dogs, however, we HIGHLY recommend that your dog be immunized & kept up to date with their Bordetella (Kennel Cough) vaccine. Keeping your dog current with their immunization will help to prevent the spread of kennel cough and/or diminish the severity of infection in your pet. With the recommendation that all dogs visiting us be vaccinated against kennel cough, if a pet should become infected, all treatment and costs associated shall be the pet owner’s responsibility. We care about the health of your pet.
Fleas/Ticks Pooches N’ Purrs strives to be a flea-free salon. It is the pet owners responsibility to keep pets on a flea/tick preventative. Fleas come from many different sources, including your own yard. If your pet has any fleas they will be given a flea bath at your expense of $20. If you do not want them to receive one, you can reschedule your appointment after the problem is taken care of by you. Ticks will be removed free of charge and you will be notified if any are found.
Dangerous or Aggressive Animals Refusal of Services and/or Special Handling Fee Owners MUST inform Pooches N’ Purrs if your dog bites, has bitten, or is aggressive to people, other pets or specific grooming procedures. We reserve the right to refuse any services at any time. In the event that your pet is too stressed or becomes dangerous to groom, Pooches N’ Purrs has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and client will be charged a grooming fee (for what was done up until that point). Special handling fees may become necessary if your pet is difficult to groom.
Use of Muzzles Muzzling does not harm your pet and protects both the pet and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If your pet still acts in a way that is dangerous, Pooches N’ Purrs has the right to stop grooming services at any time and a service fee will be collected. We do not muzzle unless your pet gives us a reason to. Other methods are used to calm your pet, muzzling is a last resort. If you have read this release, please go to the window at the front desk and let us know. We will have a special treat for your pet. Pets that require muzzling or extra staff to help manage them will be charged at a higher than standard grooming rate. This is because pets showing aggression or acute nervousness take much longer to groom.
Interruptions During Grooming Services For the safety of the animals being groomed, as well as the professional pet groomer, it is asked that you do not interrupt the groomer during grooming. Every effort will be made to ensure your pet is groomed as safely as possible, but an excited pet can be dangerous to continue to work on. After dropping your pet off, please do not stop back in the shop until you have received a phone call from us that your pet is ready. Please allow up to 4 hours from the time you drop off your pet to the time it is finished. If you have any questions after drop off, please call us.
Matted Coats Animals with severely matted coats require extra attention. Mats in an animal’s coat grow tight, and can ultimately damage and tear the animal's skin, which provides a breeding ground for parasite infestations. Pooches N’ Purrs will not cause serious or undue stress to your pet by de-matting. Mats can be very difficult to remove and may require the pet to be shaved. Removing a heavily matted coat can cause nicks, cuts or abrasions due to skin growths trapped in the mats. Heavy matting can also trap moisture and urine near the pet's skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments, every 4-8 weeks. If your pet needs to be shaved to remove matting, by signing below, you acknowledge that you agree to this procedure, and any risk. There will be an additional charge for this process: it is time consuming and causes extra wear and tear on grooming equipment.
No Show and Same Day Cancellation Policy If for any reason you need to cancel or reschedule your pet's appointment, we require a 24 hour notice so we can replace your appointment. (Thursday appointments will need to be cancelled on Tuesdays as we are closed on Wednesdays. Monday appointments will need to be cancelled on Saturdays as we are closed on Sundays.) You will need to call 708-444-2990 (not email or text) to cancel. If you do not show up or cancel your appointment the same day, you will pay a non-refundable $40 PER PET fee. (Exceptions can be taken into consideration on a case by case basis.) If your pet misses its appointment 3 times, you will then be required to pre-pay IN FULL for all future appointments. If you miss or are more than 15 minutes late for appointments that you have pre-paid for, you WILL NOT be refunded. If you cancel within our 24 hour notice period, your pre-payment be transferred to your rescheduled appointment. Thank you.

We accept CASH, CHECKS, ZELLE QUICKPAY, CREDIT CARD (credit card has up to 3.5% bank service fee) If any payment is not received at time of pick up, we will initiate collection procedures, including the fee of the collection, along with the already owed amount.

Satisfaction Your satisfaction is important to us. If you are unhappy for any reason, and would like something adjusted, we will be happy to make any adjustments when you pick-up your pet from his/her appointment. Once, however, you take your pet home from the appointment, any return visits will be treated as a new appointment and client will be charged a grooming fee or some kind.
Late pick up fees PLEASE DO NOT MAKE ANY OTHER PLANS WHILE YOUR PET IS IN OUR CARE. We ask that you pick up your pet within one hour of receiving our call/text that your pet is ready. This includes leaving a message/text if you do not answer. We do have limited cage room and will need that room for our next customers. We respect your time and your pets stress level. As amazing and fun as our atmosphere is, its better for your pet to be with you! Note: If your pet has an accident in its cage after the 1-hour notification and has to be re-washed, there will be an additional charge for that.
